Care Management

Coping with the challenges of aging, the loss of a loved one and diminishing resources, both physical and financial, can be overwhelming. The decisions an elderly person makes to cope with a specific situation may permanently affect their independence.
Care managers arrange for services, which allow older adults to live as independently as possible, for as long as possible.
Care Management provided by the Aging Network across Nebraska, offers older adults coordinated care in the home. Care Management is a fee for service program.*
The care management client has access to services from a variety of agencies to find just what he/she needs, at an affordable cost.
Transportation for an elderly person who can't drive can be established.
House cleaning services can be arranged, or hot meals can be delivered.
Services arranged through care management may include transportation, housekeeping, home delivered meals and emergency response systems.
* Care Management is a fee for service program, however services will not be denied due to
lack of payment.
